Do all companies have Workman's compensation insurance? NO
Why do some companies not have Workman's compensation? It is far more expensive than general liability which only insures for property damage that may occur. If a worker is injured the costs are far higher and if his employer cannot cover his medical expenses the injured may expect you to pay the bill.
